Nursing Home Administrators: Background Report Instructions

Individuals interested in obtaining an New Mexico Nursing Home Administrator license must complete an Authorization for Release of Information form located HERE. The form is to be printed and completed with a $15.00 fee and mailed to:

Department of Public Safety
P.O Box 1628
Santa Fe, NM 87504

Department of Public Safety will release their finding to the Nursing Home Administrators Board, and a licensing specialist will update you on the status of your background check when complete. Please note that the board does not conduct the background checks.
